Required and Optional Forms 2023-24

Parents, 

Here are a few Back-to-School tasks:   

  1. Sign-up for the Jackson MS News You Choose
  2. Activate your ParentVUE account if you haven't already. Request a ParentVUE Activation Key.
  3. Update forms in SIS ParentVUE, must be updated yearly. These forms can be completed and submitted online through SIS ParentVUE and the new OVU. 
  4. Review all the Tech Tools available in FCPS.
  5. Engage in the Jackson MS Community through volunteering, joining the PTA, working with teachers, etc.

Required Forms for All Students:

  1. Emergency Care Form

    • All students must have accurate emergency care information each school year. Parent/Legal Guardians update this information through WeCare@School via their SIS ParentVue account. 

  2. Health Information Form - In SIS ParentVUE.

  3. Digital Resource Consent - This parent consent allows your child to use FCPS approved digital tools that require parental consent.

      4. Intent to Ride School Bus- In SIS ParentVUE

      5. After School Program Registration Form (ALL students must complete, regardless of intent to participate).  More information about our After School Program can be found here. 

     6. SOL Retake Parent Permission Form- All parents can complete for students regardless of SOL score. 

     7. In SIS ParentVUE- Under Report Card Tabs choose "Go Paperless" 

Opt-Out Forms available through ParentVUE:

The following opt-out forms can be completed as needed by going to the “Student Info” tab and clicking “Edit Information" in ParentVUE

  • SEL Screener Participation
  • Counseling Opt Out
  • Transportation Intent
  • Home Internet Access
